Terms and Conditions:

This Camper has my permission to engage in all camp activities except as noted on the Medical Form. In the event I cannot be reached in an EMERGENCY, I hereby give permission to Bliss Summit Bible Camp to secure the proper treatment for my child named above. I give Bliss Summit Bible Camp my permission to use photos and video of normal camp activities for use in future camp promotion and publications.

Bliss Summit Bible Camp is licensed by the New York State Department of Health and inspected twice yearly with records filed at: Wyoming County Health Department 338 North Main Street Warsaw, New York 14569. A nurse or EMT is on the camp grounds 24 hours a day, and a doctor is on call for the health and safety of your child.
